OBIETTIVI DEL CORSO

Chronic spinal pain (CSP) is characterized by impaired spinal motor control, maladaptive pain cognitions and brain alterations (e.g., decreased brain matter density). Evidence-based clinical guidelines for CSP management advocate the use of exercise programmes1. Exercise therapy for improving spinal motor control is effective in reducing pain and disability related to CSP2-4, but the effects are similar to those seen in response to general exercise therapy5,6, and effect sizes are rather small5,6. These findings make us think further, beyond muscles or consider that exercises need to be better tailored to address the impairments of each individual. Many clinicians combine the unique skills for targeting the CSP patient’s mind, body and brain concomitantly7-11. Yet clinicians are often unaware of their ability to treat complex patients with CSP pain. Therefore, the course aims at learning clinicians applying evidence-based guidelines for CSP management. The course will first provide evidence based guidelines for the treatment of CSP, followed by the recent developments of exercise therapy for CSP discussed together with neuroscientific findings of nociception-motor interactions. Next, a clinical guide on how to address maladaptive pain cognitions during exercise therapy for CSP patients is provided. Next, the 2 worlds will join together, explaining that joining forces by providing exercise therapy for improving pain cognitions and impaired motor control8 generates a synergistic effect. This will be illustrated with findings from a recently completed randomized clinical trial12, showing that such a combined approach not only imparts clinically significant improvements in pain and disability, but also results in central nervous system changes.  To facilitate active involvement of the audience, the course will emphasize skills training for clinicians rather than lecturing. Hence, the course focus will be on teaching the follow skills that clinicians can directly apply in daily clinical practice:

  • classifying CSP patients as having either predominant nociceptive, neuropathic or central sensitization neck or low back pain;
  • clinical reasoning skills in people with CSP;
  • communication skills to explain pain to people with CSP;
  • communication skills applied when providing exercise, manual therapy or behavioural interventions for people with CSP;
  • designing a comprehensive treatment plan for people with CSP;
  • providing cognition-targeted exercise therapy for people with CSP;
  • applying motivational interviewing skills for people with CSP.
